Business summit Award applications
Congratulations on your nomination for the 2012 Columbus Chamber Business Summit Awards. These prestigious awards are presented by the Columbus Chamber and its Small Business Council.
The Columbus Chamber Business Summit Awards recognize outstanding individuals in the following categories: Adapt & Innovate, Diversity Leadership, Emerging Excellence and Young Professional Leadership.
To be fully considered for the Business Summit recognition, please provide detailed information about your company through our online application process. All information provided will be held in the strictest confidence by the Columbus Chamber and the Business Summit Award judges.
Register for the Columbus Chamber Business Summit on November 13, 2012.
To be included in the final consideration of this award, your completed application must reach the Columbus Chamber no later than 5 p.m., Thursday, September 20, 2012.
The registration form is in a user-friendly PDF format. Type or paste your answers in the boxes provided, and print or email the application for submission. NOTE: A full version of Adobe Acrobat is needed in order to save your work periodically and return to complete it at a later time.
The application process will occur in two phases. In the first phase, any business may apply for a Columbus Chamber Business Summit. In the second phase and once applications have been received and reviewed by the selection committee, a list of finalists will be generated and made public. Finalists will be required to submit their biography, photo and corporate logo.
This information will be used in announcements of the event finalists and award recipients. Finalists will be announced after Friday, October 19.

Business size standards to qualify as a small business are set by industry and based on the average number of employees for the preceding 12 months or on sales volume average over a three-year period. These standards are available through the Small Business Administration website: http://www.sba.gov.
Membership in the Columbus Chamber is not required to apply or to be selected as a recipient.
